Overview
A well-established, physician-led medical group with multiple locations across Long Island is seeking an Internal Medicine Nurse Practitioner to join its 100% outpatient practice. Known for clinical excellence, strong patient satisfaction, and multidisciplinary collaboration, the group offers a supportive environment for a provider ready to deliver high-quality primary care and help grow a thriving patient panel.
What you'll do
- Serve as a key clinical provider in the delivery of high-quality care
- Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management
- Refer patients to appropriate specialty services as needed
- Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies
- Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system
- Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care
- Provide clinical oversight to nurses, technologists, and support staff within the practice
- Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines
- Participate in population health initiatives and support ongoing quality improvement projects
- Contribute to the development of a sustainable patient panel aligned with practice growth objectives
Required qualifications
- BSN and MSN from an accredited nursing school
- NYS RN & NP license, BLS, Board Certified
- Trained in Internal Medicine
- 8+ years experience as a NP
- Strong diagnostic, communication, and interpersonal skills
- Team-oriented with a patient-centered approach to care
- Proven ability to maintain and grow a local clinical practice is required
- Entrepreneurial mindset with an ability to grow and manage a thriving patient base
Preferred qualifications
- Familiarity with EMRs and value-based care metrics is a plus
