Overview

A physician-led outpatient medical group with multiple locations across Long Island is seeking an Internal Medicine Nurse Practitioner. This is a 100% outpatient role based in Coram, NY, offered as part time or full time with flexible hours Monday–Friday and no call.

What you'll do

• Serve as a key clinical provider delivering high-quality patient care
• Provide direct outpatient primary care, including preventive, acute, and chronic disease management
• Refer patients to appropriate specialty services as needed
• Educate patients on diagnoses, treatment options, and preventive strategies
• Maintain thorough, accurate documentation in the Electronic Medical Record (EMR) system
• Collaborate with referring physicians, specialists, and care teams for seamless patient care
• Provide clinical oversight to nurses, technologists, and support staff
• Maintain compliance with HIPAA, NYS DOH, CMS, and relevant payer guidelines
• Participate in population health initiatives and quality improvement projects
• Contribute to building a sustainable patient panel aligned with practice growth goals

What you'll need

• BSN and MSN from an accredited nursing school
• NYS RN and NP license, BLS, Board Certified
• Training in Internal Medicine
• 8+ years of experience as a NP
• Strong diagnostic, communication, and interpersonal skills
• Team-oriented, patient-centered approach to care
• Proven ability to maintain and grow a local clinical practice
• Entrepreneurial mindset with ability to grow and manage a thriving patient base
• Familiarity with EMRs and value-based care metrics is a plus
