Overview

A premier health organization in the Finger Lakes region of New York is seeking a dedicated Outpatient Physical Therapist to join its rehabilitation services team. This role offers the chance to provide high-quality, patient-centered care in an outpatient clinic setting while contributing to a collaborative, growth-oriented team culture.

What you'll do

• Conduct initial evaluations and develop individualized treatment plans tailored to each patient's needs
• Administer therapeutic treatments including exercises, manual therapy, modalities (ultrasound, heat/ice therapy, electrical stimulation), and patient education
• Educate patients on exercise programs, posture correction, and injury prevention for long-term recovery
• Collaborate with a multidisciplinary care team to deliver comprehensive, patient-centered care
• Monitor patient progress and adjust treatment plans as needed to meet rehabilitation goals
• Document treatment plans, progress notes, and discharge summaries in compliance with departmental policies and industry standards
• Provide patient education to empower individuals in managing rehabilitation and preventing future injuries
• Maintain a clean, safe, and organized environment, adhering to infection control and safety guidelines

Required qualifications

• Must hold or be eligible to obtain a valid Physical Therapist license in the state at time of hire
• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program required
• Basic Life Support (BLS) certification required within 90 days of hire
• Strong communication and collaboration skills with patients, families, and team members
• Strong organizational and time-management skills
• Ability to deliver high-quality, compassionate care to patients with orthopedic, neurological, and musculoskeletal conditions
• Must be able to lift up to 30 pounds independently and up to 150 pounds with assistance
• Ability to perform frequent standing, walking, bending, and moving patients during treatment sessions

Preferred qualifications

• Prior experience in outpatient physical therapy settings preferred
