Location: Scranton, PA | Schedule: Full-time, on-site/field-based

Overview

An established home health and hospice care provider is seeking a Registered Nurse to join its clinical team in Scranton, PA. This role is essential to delivering high-quality, compassionate nursing care to patients in their homes, coordinating with a multidisciplinary team to ensure comprehensive, individualized treatment plans.

What you'll do

• Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team, including physicians, therapists, social workers, and aides
• Provide direct skilled nursing care such as wound care, medication management, IV therapy, and symptom management, including pain and comfort care in hospice settings
• Educate and provide emotional support to patients and their families or caregivers based on individual needs
• Supervise LPNs/LVNs, home health aides, and other support staff
• Participate in on-call rotations, admissions, or urgent response visits as needed
• Coordinate care activities, including visit scheduling and documentation, to ensure regulatory compliance
• Monitor patient progress, respond to changes in condition, and adjust care plans accordingly
• Maintain accurate clinical records in accordance with agency policies, professional standards, and regulatory requirements

Required qualifications

• Graduate of an approved school of professional nursing
• Currently licensed to practice as a Registered Nurse in the state of agency operation
• One (1) year experience as a RN
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
• Ability to travel to all business locations
