Overview
An established home health and hospice care provider is seeking a Registered Nurse to join its clinical team in Scranton, PA. This role is essential to delivering high-quality, compassionate nursing care to patients in their homes, coordinating with a multidisciplinary team to ensure comprehensive, individualized treatment plans.
What you'll do
- Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs
- Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team, including physicians, therapists, social workers, and aides
- Provide direct skilled nursing care such as wound care, medication management, IV therapy, and symptom management, including pain and comfort care in hospice settings
- Educate and provide emotional support to patients and their families or caregivers based on individual needs
- Supervise LPNs/LVNs, home health aides, and other support staff
- Participate in on-call rotations, admissions, or urgent response visits as needed
- Coordinate care activities, including visit scheduling and documentation, to ensure regulatory compliance
- Monitor patient progress, respond to changes in condition, and adjust care plans accordingly
- Maintain accurate clinical records in accordance with agency policies, professional standards, and regulatory requirements
Required qualifications
- Graduate of an approved school of professional nursing
- Currently licensed to practice as a Registered Nurse in the state of agency operation
- One (1) year experience as a RN
- Must possess and maintain valid CPR certification while employed in a clinical role
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Ability to travel to all business locations
