Overview
A community-based healthcare organization serving the Chehalis, WA area is looking for an experienced Advanced Registered Nurse Practitioner (ARNP) to join its team-based, Patient Centered Medical Home practice. This provider will work alongside physicians, ARNPs, PAs, and nursing staff to deliver comprehensive, patient-focused care to a diverse community population.
What you'll do
- Conduct advanced clinical assessments and manage conditions within ARNP scope of practice
- Apply independent, evidence-based clinical judgment to guide patient care decisions
- Partner with physicians, PAs, ARNPs, and nursing staff on coordinated care management
- Build individualized teaching plans that help patients reach and sustain optimal health
- Keep chart documentation accurate and complete within a 48-hour window, 95% of the time
- Finalize fee slips with properly matched diagnosis and procedure codes
- Meet productivity standards of at least 3,400 encounters per year on a full-time basis
- Join daily huddles to support care coordination under the PCMH model
- Flag at-risk patients and track population health trends across assigned panels
- Guide patients through self-management education and discharge planning
- Walk patients through procedures and treatment plans before they occur
- Engage in quality management, peer review, and ongoing practice development
- Help train other healthcare professionals, including student nurse practitioners
- Stay current on required annual training, including HIPAA, Cultural Diversity, BBP, and Sexual Harassment
Required qualifications
- Graduate of an accredited ARNP program
- Current RN and ARNP licensure in Washington State
- Prescriptive authority granted by Washington State
- Background working in a broad primary care clinic setting
- Solid grasp of the nursing process: assessment, planning, implementation, and evaluation
Preferred qualifications
- Experience performing minor surgical techniques
