Overview
A growing occupational health clinic operating a satellite site in Albany, NY is seeking a Social Worker to join its clinical team. This role supports patients navigating workplace injury, return to work planning, and connection to community and behavioral health resources.
What you'll do
- Conduct psychosocial assessments for patients seen through occupational health services
- Support return to work planning in coordination with providers, employers, and case managers
- Connect patients to community resources, behavioral health services, and financial or social supports as needed
- Assist with care coordination for complex or high risk occupational injury cases
- Document assessments and interventions in the EMR in line with clinic and regulatory standards
- Serve as a liaison between patients, providers, and employer or insurance contacts when psychosocial factors affect recovery or return to work
Required qualifications
- Master's degree in Social Work (MSW) required
- Strong assessment and care coordination skills
- Comfortable collaborating with medical providers and employer partners
- Excellent communication and documentation skills
Preferred qualifications
- LCSW preferred; LMSW considered
- Experience in occupational health, workers compensation, disability, or a related medical setting preferred
Compensation and benefits
- Market rate in the Albany, NY area runs approximately $65,000 to $95,000 per year ($31.25 to $45.67 per hour, depending on experience)
- LCSW licensure and occupational or medical setting experience trends toward the higher end of the range
