Overview

A Medicare-certified home health provider is seeking an experienced Home Health RN Case Manager to serve patients throughout Fort Worth and the surrounding Tarrant County area. This field-based role is ideal for a clinically strong Registered Nurse with recent home health case management experience who can manage an assigned patient caseload independently while delivering high-quality skilled nursing care in the home setting.

What you'll do

• Manage an assigned home health patient caseload in the field
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team
• Complete accurate, timely clinical documentation, including OASIS and visit notes
• Monitor patient progress, identify changes in condition, and communicate updates appropriately
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership
• Maintain compliance with home health regulatory, documentation, and quality standards

Required qualifications

• Active Texas RN license in good standing
• Recent Home Health RN Case Management experience required
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing
• Ability to work independently while maintaining strong communication with the clinical team
• Strong clinical judgment, organization, documentation, and time-management skills
• Valid driver's license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County

Preferred qualifications

• EMR experience preferred
