Overview
A community health center in the Greater Sacramento area, CA (multiple locations available) is seeking a full-time Nurse Practitioner or Physician Assistant to deliver comprehensive primary care to diverse and underserved populations. This on-site role works collaboratively with physicians and multidisciplinary care teams, with a schedule of Monday to Friday 8-5 PM or a 4-day, 10-hour shift option.
What you'll do
- Perform comprehensive patient evaluations, including history and physical examinations
- Diagnose and manage acute and chronic medical conditions
- Order and interpret laboratory tests, imaging, and other diagnostics
- Prescribe medications and therapeutic interventions within scope of practice
- Develop and implement treatment plans in collaboration with the Medical Director
- Monitor patient progress and modify care plans as appropriate
- Initiate emergency procedures when clinically indicated
- Provide preventive care, health education, and counseling services
- Maintain accurate and timely documentation in accordance with agency and regulatory standards
- Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care
What you'll need
- Active California RN and Nurse Practitioner license
- Current Nurse Practitioner board certification
- Current DEA certification
- Current BLS certification
- 1–2 years of clinical experience preferred, particularly in community health or underserved populations
- Strong commitment to serving low-income, homeless, or medically underserved communities
- Comfortable treating three patients per hour
- EPIC experience
- In-basket management competency
- Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred
Compensation and benefits
- $131,000 – $170,000 annually (based on experience)
