Hospice RN Case Manager

Overview
A respected hospice care provider is seeking a full-time RN Case Manager to coordinate and oversee patient care in the Kansas City metro area. This role is central to delivering compassionate, individualized care to hospice patients and their families through clinical expertise and close interdisciplinary collaboration.
What you'll do
- Coordinate and oversee all aspects of assigned patients' hospice care
- Develop and implement individualized plans of care with the interdisciplinary team
- Perform head-to-toe assessments, monitor symptoms, and manage medications
- Educate patients and families regarding condition, needs, and care
- Communicate patient progress with clinical leadership, physicians, and the care team
- Complete all required documentation accurately and on time
Required qualifications
- Current Registered Nurse license valid for practice in Missouri
- Current driver's license
- Current CPR certification
- Two years of Registered Nurse experience in a clinical care setting
- Reliable transportation and valid automobile liability insurance
- Ability to work independently and make sound clinical decisions
- Strong written, verbal, organizational, and interpersonal skills
Preferred qualifications
- One year of full-time hospice experience preferred
