Overview
A home health care provider is seeking a compassionate and skilled Physical Therapist to join its team, delivering personalized, in-home care to a diverse patient population. This full-time role offers the chance to help patients achieve their mobility and health goals in the comfort of their own homes.
What you'll do
- Provide patient care and document treatment in accordance with physician orders, agency policy, and patient needs
- Perform patient evaluations according to agency policy and procedure, selecting appropriate diagnostic tools to assess all aspects of care
- Inform the physician of assessment results and recommended clinical interventions prior to initiating care
- Develop and document treatment plans, including selecting intervention approaches, strategies, materials, and treatment priorities
- Deliver organized, efficient patient treatment including advising, educating, and counseling patients
- Present pertinent patient information to physicians and keep team members informed of patient status and treatment plan
- Provide appropriate treatment according to patient needs and within agency and payer parameters
- Determine objective, measurable goals in collaboration with team members and patients
- Meet agency productivity standards and use work hours efficiently
Required qualifications
- Minimum nine months experience as a physical therapist
- Master's degree in physical therapy from a CAPTE accredited physical therapist education program
- Must be a licensed physical therapist within the state of agency operation, compliant with accepted professional standards and practices
- Valid CPR certification maintained while employed in a clinical role
- Licensed driver able to travel to all business locations
Preferred qualifications
- Community/home health experience preferred
