Location: Burnsville, MN | Schedule: Full-time, in-home/field-based

Overview

A home health care provider is seeking a compassionate and skilled Registered Nurse to deliver individualized patient care in the home setting. This role is central to ensuring patients receive high-quality, coordinated clinical support while working closely with an interdisciplinary care team.

What you'll do

• Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs
• Collaborate with the interdisciplinary team to develop, implement, and evaluate personalized care plans
• Provide skilled nursing interventions including wound care, medication management, IV therapy, and symptom/pain control
• Educate patients and family members on care needs and health management
• Deliver emotional support tailored to individual patient and caregiver needs
• Supervise LPNs/LVNs and home health aides while coordinating care activities and documentation
• Respond to urgent patient needs, participate in admissions and on-call rotations, and adjust care plans based on ongoing monitoring
• Maintain accurate, compliant clinical documentation while ensuring compassionate care delivery

Required qualifications

• Graduate of an approved school of professional nursing
• Current license to practice as a Registered Nurse in the state of agency operation
• One (1) year experience as a RN
• Valid CPR certification, maintained throughout employment in a clinical role
• Licensed driver with an insured automobile in good working order, meeting state and/or organizational requirements
• Ability to travel to all business locations
