Overview

A home health care provider is seeking an Occupational Therapist to deliver patient-centered therapeutic care to an assigned caseload of patients in coordination with physicians and the broader care team. This role is central to helping patients achieve functional goals, stay engaged in their recovery, and have a positive care experience within the home health setting.

What you'll do

  • Manage the complete occupational therapy care process for an assigned group of home health patients
  • Complete patient assessments and participate in developing and implementing individualized care plans focused on goal achievement and patient engagement
  • Provide or direct the majority of a patient's occupational therapy services throughout the home health continuum of care
  • Promote patient and family education using verbal and written communication techniques tailored to cultural, ethnic, and personal needs and preferences
  • Collaborate with patients and families to teach appropriate self-care techniques
  • Submit documentation in a timely manner per agency policy
  • Maintain knowledge of current federal, state, and agency regulations concerning patient care, including incident management and reporting
  • Maintain simple records in English using basic computer literacy

Required qualifications

  • 1 Year Home Health Experience
  • Bachelor of Science in Occupational Therapy from an accredited program
  • Successful completion of the National Certification Examination for Registered Occupational Therapist
  • State license in practicing state
  • Reliable transportation and proof of valid automobile liability insurance
  • Valid driver's license
  • Graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association
  • Comprehensive knowledge of general therapy theory and practice with ability to work independently
  • Computer literacy with ability to maintain simple records in English

Preferred qualifications

  • At least one year of Home Health Care experience preferred
  • Preferably a member of the American Occupational Therapy Association
