Overview
A physician-led, multi-site medical practice serving patients across Long Island is seeking a Family Nurse Practitioner for its outpatient primary care team. This is an on-site role based in Plainview, NY, offered as full time or part time with flexible Monday–Friday hours and no call responsibilities.
What you'll do
- Serve as a key clinical provider in the delivery of high-quality patient care
- Deliver direct outpatient care, including preventive, acute, and chronic disease management
- Refer patients to appropriate specialty services as needed
- Communicate clearly with patients about diagnoses, treatment options, and preventive strategies
- Maintain thorough, accurate documentation within the EMR system
- Collaborate with referring physicians, specialists, and care teams to ensure coordinated care
- Provide clinical oversight to nurses, technologists, and support staff
- Maintain compliance with HIPAA, NYS DOH, CMS, and relevant payer guidelines
- Participate in population health initiatives and quality improvement projects
- Contribute to building a sustainable patient panel aligned with practice growth goals
What you'll need
- BSN and MSN from an accredited nursing school
- NYS RN & NP license, BLS, Board Certified
- Strong diagnostic, communication, and interpersonal skills
- Team-oriented, patient-centered approach to care
- Proven ability to maintain and grow a local clinical practice
- Entrepreneurial mindset with ability to grow and manage a thriving patient base
- Familiarity with EMRs and value-based care metrics is a plus
- 8+ years of experience as a Nurse Practitioner
Compensation and benefits
