Location: Plainview, NY | Schedule: Full time or Part time, Monday–Friday, no call | Compensation: $200K-$300K per year

Overview

A well-established, physician-led outpatient medical group with multiple locations across Long Island is seeking a Family Nurse Practitioner to join its team. Known for clinical excellence, strong patient satisfaction, and multidisciplinary collaboration, this 100% outpatient practice offers a chance to deliver high-quality primary care in a supportive, growth-oriented environment.

What you'll do

• Serve as a key clinical provider in the delivery of high-quality care
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management
• Refer patients to appropriate specialty services as needed
• Communicate clearly with patients and educate them on diagnoses, treatment options, and preventive strategies
• Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care
• Provide clinical oversight to nurses, technologists, and support staff within the practice
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines
• Participate in population health initiatives and support ongoing quality improvement projects
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives

Required qualifications

• BSN and MSN from an accredited nursing school
• NYS RN & NP license
• BLS certification
• Board Certified
• Strong diagnostic, communication, and interpersonal skills
• Team-oriented with a patient-centered approach to care
• Proven ability to maintain and grow a local clinical practice
• Entrepreneurial mindset with an ability to grow and manage a thriving patient base
• 8+ years experience as a NP

Preferred qualifications

• Familiarity with EMRs and value-based care metrics is a plus
